	Lunch Room Assistant Volunteer Form
	If you are interested in volunteering as a Lunch Room Assistant for the 2009-2010 school year, please submit this form no later than Friday, September 11th.
	Our Healthy Lunch Program has been a success and will continue as long as we have the volunteers to make it happen! The smiles and thanks you will receive from the children is a priceless reward.
	We are asking for a 2-hour and 15 minute lunch commitment on a weekly or bi-weekly basis from all volunteers. Please understand volunteers must work the lunch period and not just their child’s/children's lunch time.
