Soccer Program Registration Form
Fall 2012
All boys and girls grades 1-8 in September are invited to join a Trinity Academy soccer team during the upcoming season. There will be a multi-session instructional clinic for Kindergarten children. If you are interested, please submit the online form by FRIDAY, JUNE 15th. Please submit registration form online or to the office along with a signed MEDICAL RELEASE/CODE OF CONDUCT FORM by this date. Forms submitted after June 15th will not guarantee a spot on the roster. Late registrations will only be used to fill gaps on teams on an "as needed" basis.
FEE: $70 per child
NOTE: A signed Medical Release/Code of Conduct Form is required for each registered player. Please print and return the completed form to the office by the registration date.
Please note that your registration is not official until payment is received. If you prefer to pay by check, please make checks payable to: TRINITY ACADEMY HSA. Refunds will only be issued until teams have been determined, after which no refunds will be issued.
